Résumé
La xénotransplantation a été dès le début du XX e siècle, la première preuve de concept de la faisabilité technique de la transplantation d’organe, par la mise au point des anastomoses vasculaires. Mais jusque peu, elle a bien entendu toujours été confrontée à la problématique de la discordance d’espèce menant à des rejets immédiats ou très précoces. La meilleure compréhension des phénomènes engagés, combinée d’une part à l’accès aux techniques de modification génétique chez les grands animaux et en particulier le porc et d’autre part à de nouvelles immunosuppressions, ont permis de franchir de nouvelles étapes dans les modèles précliniques porcs sur primates non humains avec des prolongations de survies, telles que l’avancée majeure du passage chez l’homme s’est produite depuis l’année 2021. À ce stade, les expérimentations humaines demeurent exceptionnelles et pilotes, essentiellement aux États-Unis et à un moindre niveau en Chine et l’observation de leurs résultats sera d’un grand enseignement. Ces « premières » montrent certainement la voie pour le redémarrage d’autres dynamiques, notamment françaises et européennes.
Summary
Xenotransplantation, from the beginning of the 20th century, was the first proof of concept for the technical feasibility of organ transplantation, through the development of vascular anastomoses. But until recently, it has, of course, always faced the problem of species discordances leading to immediate or very early rejection. A better understanding of the phenomena involved, combined on the one hand with access to genetic modification techniques in large animals, particularly pigs, and on the other hand with new immunosuppression, has made it possible to take new steps in preclinical pig models on non-human primates with prolonged survival, such that the major advance in the transition to humans has occurred since 2021. At this stage, human experiments remain exceptional and pilot, mainly in the USA and to a lesser extent in China, and observing their results will be very instructive. They certainly show the way for restarting other dynamics, particularly in France and Europe.
