Résumé

L’observance dans les traitements à long terme est analysée du point de vue de l’intentionnalité, c’est-à-dire du rôle de la formation d’une intention dans la décision d’accomplir un geste thérapeutique comme, par exemple, prendre un comprimé. Nous montrons que la non-observance peut être intentionnelle, conséquence d’une décision active du patient (ici, l’intention étant de ne pas prendre le comprimé), et non intentionnelle, phénomène passif qui ne dépend pas de sa volonté, comme, par exemple, le simple oubli. Inversement, l’observance est le plus souvent intentionnelle, le but étant d’éviter les complications de la maladie. Mais elle peut aussi ne pas être précédée par la formation d’une intention et être accomplie par habitude, conduisant à proposer le concept d’observance non intentionnelle. Cette réflexion sur la place de l’intentionnalité dans l’observance a des implications pratiques et éthiques.

Summary

Adherence in long-term therapies is analysed from the point of view of intentionality, i.e. the role of the formation of an intention in the decision to carry out a therapeutic action such as taking a tablet. We show that non-adherence can be intentional, the consequence of an active decision by the patient (here, the intention being not to take the tablet), and non-intentional, a passive phenomenon that does not depend on his or her will, such as simple forgetfulness. Conversely, adherence is most often intentional, the aim being to avoid complications of the disease. But it can also be unintentional and performed out of habit, leading to the concept of non-intentional adherence. This reflection on the place of intentionality in adherence has practical and ethical implications.
