Résumé

L’hypercholestérolémie familiale est une maladie génétique autosomique dominante liée à une mutation sur un des trois gènes impliqués dans le catabolisme des particules LDL (Low Density Lipoprotein) : LDL récepteur, apolipoprotéine B et proprotéine convertase subtilisine/kexine de type 9 (PCSK9). La prévalence est estimée entre 1 pour 500 naissances à 1 pour 250 quand le diagnostic est respectivement phénotypique et génétique. Du fait de l’élévation du LDL-cholestérol depuis la naissance et du niveau élevé (double de la normale dans la forme hétérozygote), l’exposition est très importante et explique le risque cardiovasculaire particulier de cette hypercholestérolémie. Le diagnostic est souvent fait tardivement au moment du premier accident cardiovasculaire. Un score clinique validé par une équipe hollandaise permet d’établir une probabilité diagnostique. Le traitement comprend les mesures hygiéno-diététiques et la correction de tous les facteurs de risques ainsi qu’un traitement précoce par statine si nécessaire associé à l’ezetimibe. La place des nouvelles thérapeutiques reste à déterminer mais les anticorps anti PCSK9 représentent une importante perspective pour les patients les plus graves.

Summary

Familial hypercholesterolemia is an autosomic dominant hereditary disease, caused by mutations in genes involved in the LDL (Low Density Lipoprotein Cholesterol) catabolism: LDL receptor, apolipoprotein B and Proprotein convertase subtilisin/kexin type 9 (PCSK9). The prevalence of the heterozygous form has been estimated between 1/250 and 1/500 by phenotype or genotype, respectively. In these patients, LDL-cholesterol levels are genetically elevated since foetal life and the long term exposure of the arteries to high level of blood cholesterol leads to premature atherosclerosis. Familial hypercholesterolemia often goes unnoticed since its clinical expression is almost non-existent before the first cardiovascular episode which is often premature in untreated patients. Dutch lipid clinic criteria is a composite score used in Europe allowing to classify patients in probability groups of being a case of heterozygous familial hypercholesterolemia, from possible to definite diagnosis. Therapeutic management consists in lifestyle modifications (smoking cessation, lose weight, physical activity and low-cholesterol diet), and in a lifelong drug treatment (statins for all familial hypercholesterolemia patients and high doses of statins in adults) often in combination with ezetimibe. The target population for PCSK9antibodies remains to be established. These drugs represent a major hope for the most severe subgroup of patients suffering familial hypercholesterolemia.
