La formation en médecine tropicale en France

Résumé

La médecine tropicale a été un élément déterminant dans la structuration médicale apportée par la France dans nos anciennes colonies puis plus tard dans les pays qui s’inscrivaient dans le champ de la coopération. Au cours des dernières décennies, notre pays a réduit drastiquement l’engagement bilatéral qu’il apportait aux pays de la zone tropicale et plus particulièrement aux pays d’Afrique subsaharienne. Dans le même temps, l’enseignement de la médecine tropicale qui était référencé et reconnu hors de nos frontières perdait son expertise. Initialement portée par quelques grands centres français disposant chacun d’enseignants ayant une expertise entretenue du terrain, l’offre des formations en médecine tropicale s’est depuis démultipliée au sein de chaque université. Les programmes et leurs objectifs pédagogiques priorisés sur les maladies infectieuses et parasitaires ne répondaient plus aux priorités de santé de pays du sud désormais confrontés à une transition épidémiologique et à l’essor des maladies non transmissibles. Les enseignants porteurs de ces formations n’avaient pour un grand nombre aucune expertise reconnue en médecine tropicale. Cette évolution n’a pas été sans conséquence sur les programmes de recherche en médecine et santé tropicales et la lisibilité de notre action au profit des pays en développement.

Dans ce contexte, les  perspectives suivantes doivent être envisagées :

1-La formation en médecine tropicale doit être valorisée par l’octroi d’un diplôme national reconnu par les instances internationales. 2- La création d’un cursus doctorant en médecine tropicale est le préalable pour atteindre cet objectif avec une composante recherche importante dans ce futur diplôme.3.L’enseignement en médecine tropicale doit redevenir pratique et être assuré par des enseignants ayant une expérience entretenue du terrain. 4-La formation en médecine tropicale doit s’inscrire dans une relation bilatérale avec les pays concernés où chaque partie pourrait apporter sa contribution dans un partenariat respectant les spécificités de chacun .5- La formation en médecine tropicale doit être adossée à une recherche scientifique de haut niveau en s’appuyant sur une synergie renforcée de nos réseaux actuels (Institut de Recherche et de Développement, réseau des instituts Pasteur d’outre-mer, universités…). 6. L’enseignement de la médecine tropicale doit être coordonné par une structure supra universitaire dont la mission serait d’apporter une cohérence au dispositif actuel. 7- La réussite dans tous ces objectifs implique un engagement politique fort, seul moyen pour redonner à notre pays la place enviée qu’il occupait dans un passé récent.

Summary

Tropical medicine was a key element of the medical structures provided by France to our former colonies and, later, to countries within the scope of our international cooperation. In recent decades, France has drastically reduced its bilateral commitments to countries in the tropics, and especially in sub-Saharan Africa. At the same time, the teaching of tropical medicine, which was highly regarded even beyond our borders, has lost a good deal of its expertise. Initially available in a few large French centers, and ensured by teachers with extensive field experience, training in tropical medicine is now offered in many universities. However, their programs and educational objectives, focusing mainly on infectious and parasitic diseases, no longer meet the healthcare priorities of southern countries, which are facing an epidemiological transition and the rise of non communicable diseases. Few teachers now have recognized expertise in tropical medicine. These changes have had negative consequences for research programs in tropical medicine and for the image of French assistance to developing countries.

In this context, the following perspectives should be considered:

1) training in tropical medicine should be enhanced by the creation of a national diploma recognized by international bodies. 2) The creation of a doctoral course in tropical medicine is a prerequisite for achieving this goal, and the future diploma must include a significant research component. 3) Teaching in tropical medicine must become more practical and be ensured by teachers with extensive field experience. 4) Training in tropical medicine should be part of a bilateral relationship with countries in the tropics, each party contributing its expertise while respecting that of its partners. 5) Training in tropical medicine should be backed up by high-level scientific research based on enhanced synergy of our current networks (Institute for Research and Development, Network of overseas Pasteur Institutes, universities, etc.). 6) Teaching of tropical medicine should be coordinated by a supra-university structure whose mission is to ensure coherence. 7) Success in all these objectives will require strong political commitment. This is the only way to restore the enviable position which France occupied in the recent past.
