Résumé

La vision de la schizophrénie s’est totalement transformée au cours des dernières décennies. Longtemps considérée comme une pathologie au pronostic inéluctablement péjoratif, il est maintenant admis que l’évolution peut s’avérer souvent favorable. Parallèlement, le concept de staging a fait son introduction dans le champ de la psychiatrie, distinguant plusieurs stades cliniques comme autant d’étapes distinctes répondant à une physiopathologie et des prises en charges spécifiques. L’intérêt s’est alors porté sur les phases précoces de la schizophrénie, notamment la phase prodromique des signes annonciateurs de la maladie. Dès lors, une caractérisation précise des symptômes prodromiques s’est établie. L’ambition est non seulement de débuter la prise en charge au plus tôt afin de préserver le pronostic, mais également de cerner les sujets à risque qui développeront la maladie, c’est-à-dire qui réaliseront une transition psychotique. L’identification de marqueurs de transition psychotique constitue ainsi une préoccupation centrale et continue des cliniciens et des chercheurs. Plusieurs aspects liés au comportement ont été décrits et ont pu être associés à une valeur prédictive tels que des symptômes négatifs, des troubles de la pensée, le déclin du fonctionnement social ou encore les troubles du contrôle de l’agressivité. Il est intéressant de remarquer que certaines particularités des symptômes basiques, caractéristiques de la phase prodromique, rejoignent les descriptions physiopathologiques des auteurs plus anciens.

Summary

The outlook on schizophrenia has drastically changed during the last decades. For a long time considered as a pathology with an ineluctable negative prognosis, it is now admitted that the evolution can often be favorable. Meanwhile, the concept of staging was introduced in the field of psychiatry, distinguishing several clinical stages as distinct physiopathological stages with specific treatments. Interest was especially focused on the early phases of schizophrenia, notably the prodromal phase. Since then, a precise characterization of the prodromal symptoms has been established. The ambition is not only to start treatment as soon as possible in order to preserve prognosis, but also to identify subjects at risk who will develop the disease, i.e. who will undergo a psychotic transition. The identification of markers of psychotic transition is thus a central and ongoing concern of clinicians and researchers. Several behavioral aspects have been described and have been associated with a predictive value, such as negative symptoms, thought disorders, decline in social functioning or even disturbances in controlling aggression. It is interesting to note that certain features of the basic symptoms, characteristic of the prodromal phase, are in line with the pathophysiological descriptions of earlier authors.
