Résumé

La neuroradiologie interventionnelle (NRI) a été conçue dans les années 1960–1970, dans le but d’aider le geste neurochirurgical, par dévascularisation préopératoire des tumeurs hypervasculaires de la tête du cou et du rachis, d’où le nom initial et impropre d’embolisation. La gestation fut longue et incertaine, jonchée d’obstacles corporatistes et de critiques souvent non scientifiques, pour ne pas dire malhonnêtes. Tout au long de ce parcours difficile, le Pr. Pierre Lasjaunias a joué un rôle majeur en aidant au décryptage de la micro-anatomie des vaisseaux de la base du crâne et de leurs variantes anatomiques. L’arrivée en 1991 des coils à détachement électrolytique, pour le traitement des anévrismes cérébraux, a fait basculer la NRI vers une discipline à part entière, remplaçant au fur et à mesure de l’évolution des concepts et de la technologique, les indications thérapeutiques neurochirurgicales « traditionnelles ». Depuis l’année 2015 date de la publication de l’étude randomisée « Mr CLEAN », le traitement de l’accident vasculaire cérébral fait maintenant part intégrante de l’arsenal thérapeutique de la NRI. Le système nerveux central rejoint donc la magnifique histoire thérapeutique du cœur !

Summary

Interventional neuroradiology (INR) was conceived around 1960–1970. Primarily, the goal was to devascularise the hypervascular tumors of the head, neck and spine, in order to facilitate the neurosurgical procedures, hence the original and unsuitable name of embolization! The gestation was long and full of uncertainty, strewn with corporatist obstacles and non-scientific, not to say dishonest, criticisms. All along this difficult course, Pr. Pierre Lasjaunias has played a major role, decoding the microanatomy of the vessels of the base of the skull and their anatomical variations. When the detachable coils arrived in 1991, for the treatment of cerebral aneurysms, INR switched to a distinct discipline, which has progressively replaced the “classical” neurosurgical procedures, as the therapeutic concepts and technologies were going along. Since the year 2015, when the “Mr CLEAN” randomized study was published, the treatment of the acute ischemic stroke is now an integral part of the therapeutic INR arsenal. Therefore, the central nervous system is joining the wonderful therapeutic history of the heart!
