How to devise actions against inequalities in General Practice Health Care inside national territory

SUMMARY
Medically underserved areas, most of which are located in rural parts, are already numerous in France and are on the increase, owing to changes in medical demographics and the feminization of medicine. The causes are numerous and the National Academy of Medicine recommends a set of measures targeting : — initial and continuous physician training (training courses with rural GPs, interregional examinations, obligations for internists to occupy designated posts) ; — better information on the many existing measures intended to encourage implantation in priority areas, as well as better tools to analyze and adapt medical demographics, assistance with the creation of joint practices and medicalized care centers, creation of ‘‘ private assistant ’’ status for former trainees, and suppression of the clause preventing MDs from setting up in the same region as the physicians they replace ; — creation of a framework for new installations in underserved areas, with freedom to set up outside these areas ; 4/ finally, if none of these measures correct the situation within the next three years then binding measures will be necessary as a last resort, such as an obligation to practice in priority areas for appointed physicians, for a period of three to five years.
